2.1. Servicemembers Group Life Insurance (SGLI)
SGLI is a program that provides low-cost term life insurance coverage to eligibleservicemembers. If eligible, you are automatically issued the maximum SGLI coverage. If you qualify for SGLI, you are automatically enrolled and do not need to apply for coverage.
Servicemembers can make changes to their SGLI coverage. For example, servicemembers can decline SGLI coverage, select a lesser amount than maximum coverage, designate beneficiaries, and/or make other changes.

Use this form to request an advance insurance payment for a Servicemember or Veteran that is terminally ill. |
SGLV 8715 Application for SGLI Disability Extension |
Use this form to apply for a free extension (up to two years) of SGLI coverage if you are a totally disabled at time of discharge. |
