4.6. How to Apply

If you qualify as eligible for FSGLI, you are automatically enrolled and do not need to apply for coverage.  However, to reduce, decline, or restore FSGLI coverage, you must complete and submit SGLV Form 8286A , http://www.benefits.va.gov/insurance/forms/8286a.htm.

If you decline spousal coverage, you can later apply for coverage for your spouse by completing SGLV Form 8286A,http://www.benefits.va.gov/insurance/forms/8286a.htm, and providing proof of insurability (proof of good health).  You should contact your Personnel Office for any changes to Family SGLI coverage.
